ABSTRACT

A descriptive and traverse study was performed in patients with diagnosis of optic neuritis, that attended to the neuro ophthalmology service of the ophthalmological center, at the Santiago General Hospital Dr. Juan Bruno Zayas Alfonso , from the province of Santiago de Cuba, with less than a week of evolution of the disease, and visual acuity greater than 0.3, cooperatives and without refractive opacities, or neuro ophthalmologic diseases, during the period from December 2017 to November 2018. Ophthalmological clinical evaluations and electrophysiological studies of visual evoked potentials to pattern reversal, at diagnosis and eight weeks of evolution were carried out. Objective: To identify the relationship possible between the electrophysiological parameters obtained by visual evoked potentials with the ophthalmological clinical evaluation. Results: alterations predominantly of the parameters of latency and duration of the visual evoked potentials, in relation to the impairment of the clinical evaluations explored. Conclusions: The visual evoked potentials are an effective tool in diagnosis, and complement the clinical evaluation of patients with optic neuritis(AU)

ABSTRACT

Los comités de ética de la investigación son los encargados del proceso de revisión y evaluación de las investigaciones en general y de la vigilancia y control del cumplimiento de los principios éticos. A tales efectos, se diseñó un manual de procedimientos para el Comité de Ética de la Investigación del Hospital General Docente Dr Juan Bruno Zayas Alfonso de Santiago de Cuba, sobre la base de normativas nacionales e internacionales actualizadas, con vistas a establecer una metodología a seguir en los diferentes procesos que ejecuta dicho Comité. Para su elaboración se cumplieron 3 etapas que incluyeron: revisión documental, discusión grupal y consulta a expertos, en ese orden. Se estructuró teniendo en cuenta primeramente una serie de contenidos de orden general y luego se establecieron los diferentes procedimientos que rigen su funcionamiento. Su implementación contribuyó decisivamente a la recertificación del hospital en buenas prácticas clínicas


Ethics committees for investigation are those in charge of the review and evaluation processes of investigations in general and of the surveillance and control of ethical principles fulfillment. To such effects the procedures manual for the Ethics Committee of investigation from Dr Juan Bruno Zayas Alfonso Teaching General Hospital in Santiago de Cuba, was designed on the base of updated national and international regulations, aimed at establishing a follow up methodology in the different processes that this Committee carries out. For its elaboration 3 stages were accomplished such as: documental review, group discussion and experts consultations, in that order. It was structured taking into account first, a series of general contents and then, different procedures that lead its operation were established. Its implementation contributed decisively to the hospital recertification in good clinical practices.

Se realizó un estudio observacional, descriptivo, longitudinal y prospectivo de 28 pacientes con enfermedad cerebrovascular isquémica, ingresados en el Servicio de Enfermedades Cerebrovasculares del Hospital General Docente "Dr. Juan Bruno Zayas Alfonso" de Santiago de Cuba, desde enero hasta diciembre del 2011, con vistas a evaluar la respuesta eléctrica de la corteza cerebral contralateral al área de infarto y su papel en la recuperación motora de los afectados hemipléjicos por la lesión. La valoración de la actividad eléctrica se efectuó mediante el análisis cuantitativo del electroencefalograma digital y la de la recuperación funcional, por el índice de Barthel. En la serie la edad media fue de 67,2 años ± 2, predominó el sexo masculino y todos los pacientes presentaron alteración de la actividad eléctrica cortical, de los cuales, 39,3 % la manifestaba en el hemisferio contralateral al área de infarto, con una peor recuperación motora a los 6 meses de evolución. Por último, la afectación eléctrica de la corteza cerebral contralateral al área de infarto constituye una manifestación observada frecuentemente en pacientes con enfermedades cerebrovasculares isquémicas, lo que influye de manera negativa en su recuperación motora.


An observational, descriptive, longitudinal and prospective study of 28 patients with ischemic cerebrovascular disease, admitted in the Cerebrovascular Diseases Service of "Dr. Juan Bruno Zayas Alfonso" Teaching General Hospital in Santiago de Cuba was carried out from January to December, 2011, with the objective of evaluating the electric response from the contralateral cerebral cortex to the myocardial infarction area and its role in the motor recovery of the affected hemiplegic patients. The evaluation of the electric activity was made by means of the quantitative analysis of the digital electroencephalogram, and that of the functional recovery through the Barthel index. In the series the mean age was of 67.2 years ± 2, the male sex prevailed and all the patients presented changes of the electrical cortical activity, of which, 39.3% manifested it in the contralateral hemisphere to the myocardial infarction area, with a worse motor recovery after 6 months of the clinical course. Lastly, the electric damage of the contralateral cerebral cortex to the myocardial infarction area constitutes a frequently observed manifestation in patients with ischemic cerebrovascular diseases, what influences in a negative way in their motor recovery.

ABSTRACT

Se efectuó un estudio descriptivo longitudinal y prospectivo de 22 pacientes con ictus isquémico, que fueron ingresados en el Hospital General Docente "Dr. Juan Bruno Zayas Alfonso" de Santiago de Cuba desde enero de 2010 hasta igual mes de 2011, con vistas a establecer un pronóstico de recuperación funcional a partir del mapeo cerebral. Se realizó la valoración neurofisiológica y funcional mediante el índice de Barthel. En la serie estudiada la edad media fue de 57,2 años y predominó el sexo masculino. El mapeo cerebral se caracterizó por la presencia de actividad lenta (theta, delta) y disminución de beta y alfa en regiones corticales ipsilaterales y contralaterales al territorio afectado por el foco isquémico; asimismo, la magnitud de estas alteraciones se correlacionó estrechamente con la ganancia funcional. La disminución de energía en las bandas alfa, asociada al incremento de esta en las bandas theta y delta, de tipo global, constituyeron índices de mal pronóstico rehabilitador.


A longitudinal and prospective descriptive study was made in 22 patients with ischemic ictus that were admitted to "Dr. Juan Bruno Zayas Alfonso" Teaching General Hospital in Santiago de Cuba from January, 2010 to the same month of 2011, aimed at establishing a prognosis of functional recovery from the cerebral mapping. A neurophysiological and functional evaluation was carried out by means of Barthel index. In the studied series the mean age was 57.2 years and the male sex prevailed. Cerebral mapping was characterized by the presence of slow activity (theta, delta) and beta and alpha decreases in cortical ipsilateral and contralateral regions to the territory affected by the ischemic focus; likewise, the magnitude of these changes was closely correlated with the functional gain. The energy decrease in the alpha bands associated with its increment in the theta and delta bands, of global type, constituted rates of bad rehabilitation prognosis.
